renewal of judgment

I have an abstract of judgment, which is due to expire soon at the 10 year limit. I need to file a renewal of judgment, but am not sure what to do. The court clerk gave me form EJ-195 (Notice of renewal of judgment), but in looking it up on the net it seems I may also need form EJ-190 (Application for and renewal of judgement). Do I need both forms, and do I have to serve the paper to the judgment's debtor, assuming I can still find him?

You have fill out both documents and serve the defendant. Start trying to locate him now.
