Who can subpena someone?

Can I fill out the forms and have a person or documents subpenaed, or do I need a lawyer for that?

I would assume that there's an ongoing court case. If not, no subpoena can be issued. A lawyer may sign the subpoena, but a person acting as his/her own lawyer usually is considered the "lawyer" for the case, as well. If you're seeking consumer records -- phone bills, medical or employment records -- there are additional procedures that are required, including a "Notice to Consumer."
