Legal Question in Health Care Law in California

My grandmother recently had a stroke and is confined to a bed. My uncle has become her caregiver as he has been living in her home for as long as I can remember. My mother noticed possible signs of neglect to include numerous bedsores, unchanged diapers, etc.. What can we do to have my uncle removed as the caregiver and have my grandmother moved into an assisted living facility?
