Does my physician have permission to discuss my case with a Scientologist "auditor" without my permission?

No. I doubt many if any physicians would discuss anything with an auditor. You condition can be discussed without your stated permission if it is being done with someone reasonably related to your treatment, but not if you directly state no information should be given to a certain person.
