Which form is necessary for a person that's dying to sign in order to give another individual all rights to make any decisions as far as health issues even after death?

Durable Power of Healthcare, also known as an Advance Healthcare Directive. We use the latter because we combine this document with a Living Will.

Living Will or Health Care Directive and Durable Power of Attorney. One issue: if person is dying there may be an issue of 'duress' or 'incapacity.' If you might 'benefit' from these documents, I would consult a local attorney for guidance.
