name change form
after becoming a citizen i changed my name. i have lost the name change document and need it to get a driver's license. i have a passport and ss card, but the dmv wants that paper. what should i do?

If you legally changed your name, you should be able to obtain a copy from the courthouse in which the change of name was filed. good luck!

You can ask for a certified copy from the court that did the name change.
