Legal Question in Immigration Law in California
I'm filling out I-751, Petition to Remove Conditions on Residence form & It is asking for a sworn affidavits from individuals that have knowledge of us since our marriage.
Is there a form for this, does it have to be notarized or is it just an informal statement in handwriting?
Would like to know.

These are only needed if you don't have sufficient other evidence of your marriage being for other than immigration purposes. No, there is no set form for this. Affidavits are notarized. Affidavits can be in a person's own handwriting. An alternative to the affidavit is declaration under penalty of perjury. Declarations don't require notarization.
