H1B to H4

I want to apply a change of status from H1B to H4 using Form 539 .

The form does not allow me to provide a date on which I want this change to take effect . and even if I do provide a date ( in a letter attached with the form ) when I want this change of effect to take place will that be accepted by INS ?

Till I get the the approval notice back from INS , do I have to keep working on my H1B ?

Or as soon as I get a receipt notice that INS has accepted my package I can assume that my change of status has been accepted ?

The date will be whenever the application is received and processed. The receipt date of the application will be the priority date.
