I would apply for the I-90 along with the I-751 and include a copy of the I-90 with the I-751 if you don't have a copy of your I-551 alien registration card.

If you would like further assistance, please feel free to contact me at 818 609 1953 or [email protected].

You might file a replacement for your green card.

We suggest you to consult with an immigration attorney.

Yes, it would be a good idea to file for a replacement and also submit the I-751 form w/ the copy of the form I-90 that you sent in (and if you have a copy of the conditional LPR card before it was lost). You may contact me at : 415-387-1364 or email: [email protected]
