Medical check-up after marriage
I live in NY, my fiance is in CA, I plan to move there in mid-January. If we have a wedding in mid-December, can I start doing the INS medical exams in NY? (I have to come back for a few weeks to finish my employment & transfer my apartment) Or does it have to be one of the INS approved doctors from CA? What fee can I expect to pay (both for legal & for the doctors)? How long does it take?

as long as the doctor is an approved INS doctor, it should not matter where you have it done. I practice in CA, I can handle this for you, if you like.

You may have your medical exam done by any authorized civil surgeon. You may only schedule an appointment with an INS approved doctor. The fees vary, depending on which doctor you will see. I handle these types of cases. Please feel free to contact me regarding marrying your fiance in California.

I'll be straight up with you. The medical check-up is one of the last things to do before the final INS interview that you guys are married for real before you can get your permanent residence. Don't worry about it now! There's a bunch of other things to do first in your situation.
You've got to get married first, then have your spouse file the petition and papers to adjust to permanent residence for you. These are submitted to the INS in LA (if you're moving to SoCal), and then you won't get the INS interview until 9 or 12 months later. The medical check-up envelope is opened up and inspected at the interview (not before). If you get a check up too soon (I think they're only good for 6 months), you'll have to get another one at $150-200 cost again.
As stated, there are numerous papers to be filed and it's better to hire a competent lawyer in the area you're moving to since the paperwork is usually filed in person at the local INS office by the attorney.
