Legal Question in Immigration Law in California

I was told that if I received my permanent resident card (green card) over 30 years ago, I need to replace it. Do I need to file a USCIS I-90 form?

On the form, what does it mean: Date of Admission.

I have been trying to contact the U. S. Citizenship and Immigration Services and it is impossible to get through.

Can you help me?

Your date of Admission would be the date you received your Green Card.
