If your married but live in seperat addresses and one of them work and has medical inshuance with the company, is that spouse oblagated to have the other non working spouse on his or her inshurance?

From an insurance law perspective, no, not unless the policy or employer requires it. Insurance is a private contract between you and the insurer, and sometimes the employer acts like the insurer's agent for handling things.
