Death Claim Benefit Form
It states from the claim form that I'm entitle to asone of two named beneficiaries. Does both names have to claim the benefits.

If you and the other beneficiary each get separate shares of the proceeds, then no. It would make no sense to deny benefits to you because the other person does not claim his, cannot be found, etc. The answer may be different if the proceeds are to be paid jointly to the two of you (i.e., if the entire sum is to be paid to you and your spouse).
