this is fernandor chavezjr 562-587-4307 was auto accident injured disabled so can't work i went to hospital court # kc0-39520 other side insur no show at clerck she said to get attorney for award then i was looking for help wen on dec-28-2013 i was sent several accident reports of awards to collect need to have form from ( united legal advance) by attorney needs help or any attorney assisted or any you could assist or help to refference

I understand you are looking for a form, is that right? What is United Legal Advance? What is hospital court? Are you saying you went both to the hospital and to court and that the court case number is KC0-39520? If so, which Court did you go to?
