insurance penalties for not reporting in the first trimester
can an insurance company penalize me $400.00 for not reporting the rpegancy in the firt trimester?

I think your question leaves more questions to be asked, such as 1) what does the policy provide; 2) when did you get the policy; 3) is it a group workplace policy; 4) why didn't you disclose the pregnancy when you purchased the (assuming private) policy?
