You have an attorney write a letter to the host. If they do not respond, get a restraining order, and bring a lawsuit.

contact the attorney general's office (Feds), the district attorney (State), and lastly the Secretary of State if the company is registered in California. The secretary of state probably has a fraud hotline. Additionally you may wish to address the IP provider regarding the unlicensed use of your name. An attorny will be expensive and should be used only after you have exhausted other remedies, unless you have so much money that it is not an issue.

If your name is distinctive enough, you might have a claim for unfair competition...

If you're a celebrity, your right of publicity might be involved... but without knowing more, I'd say you've got a tough case.
