I am interested in purchasing the full rights to photographs and videos of a model. Is there a sample contract or where do I start?

Unless you have experience in purchasing copyrights and obtaining rights to publicity, you really should hire an Entertainment Attorney to draft your agreement.
As a Franchise Attorney I agree with the other attorney answer. You start with a visit to an intellectual property attorney's office - one with experience in copyrights. Consult with a good copyright or franchise attorney in your area for specific advice.

I advise against trying to make a form work for you if you are trying to acquire the rights of multiple photographers or to future work done by unknown photographers. Remember that the copyrights may belong to the photographers, not the model. A lawyer-drafted contract or series of contracts may be necessary. However, sample IP contracts and/or blank contract forms may be found in treatises at county law libraries (every county has a public law library, often at or near the courthouse).
