affair w-boss/sex. harassed by coworker/fired for retaliation

Worked for Invest. Firm almost 2.5 yrs. Prior, had relations with my married friend that came my boss later. After Hire two encounters occured then I said ''No More''. Our IT guy asked me out constantly. no thanks. Came a time when he started being rude, would start typing on my keyboard as I was working, pushing my hands out of the way. Would not stop after many requests. Escalated into a huge verbal altercation. He called spoke to my boss before I could and when I tried to report the incident, I was told I could leave if I didn't like it. I was too sensitive. I was fired a week later. They said it was for attendance, I served 90 probation for being tardy. This was after reporting my severe depression and anxiety to my employers and requesting a 30 min adjustment to my start time. Have to file with DFEH today. Should I tell them EVERYTHING?? Including the affair with my boss? I'm ashamed. This is an onion of a story.

Tell DFEH all that is necessary to support a claim of sexual harassment and discrimination. Also claim a failure by the employer to conduct a reasonable investigation.
