Is one certification per year all that is required for intermittant leave?

I have a medical condition that requires intermittant short term leave. After taking the time to obtain the certification from my physician and providing this for my employer, they are now asking for a ''Doctor's Note'' each time I need a day or a few days. Are they allowed to require a the employee to visit the physician each time when they have already received certification?

A blanket physician'n certification should be adaquate if it describes that the needed accomodation of intermittant leave is medically required and that there is no purpose to a doctor's visit for each leave because the nature of the medical condition, while requireing periodic medical treatment, does not require medical treatment for every incident. This should be adaquate for FMLA/CFRA leave (if your company is large enough) as well as for an accomodation for your disability under state and federal law.
