Finding documents in personnel file after termination

Can an employer put documents in an employee's personnel file after the employee requested copies after being terminated just to cover the employers butt? and is it legal to put "refuse to sign" when the employee had never seen such document ?

they can legally put anything they want in the file but if there is litigation they cannot lie about it and it may hurt their credibility with a jury.
