Pay raise signed & dated by supervisor not received

My former supervisor gave me a copy of a change of status form with the date and his signature, indicating I would be getting a $1.00/hr raise,effective on a specific date. I never received the raise, nor any notification or explanation as to why it had been withdrawn. This despite his comments on the form praising my excellent performance, stating I deserved ''Top Pay'', even recommending me for a part time supervisor position if one opened up. Am I entitled to back pay from the effective date on the form?

No. Unless you have a binding agreement, you will not be entitled to back wages. Have you spoken to your supervisor? You should do so to determine whether this is a payroll error.
