Unpaid Medical Leave

Recently I had a back injury (1999). The doctor placed me permanent and stationary condition, I am constantly on pain (taking pain medication), I am constanly absent from work. I tried to ask my doctor for excuse slip to cover my absences, but the doctor refused to give one and employer requires an excuse slip for absences. because of so much absences the employer placed me on Medical leave. my question is Can the Employer authorize a medical leave (unpaid)?
