verification of wages by employer

I was involved in an automobile accident this year. I signed an authorization for my insurance company to obtain wage verification from my employer. The insurance company did send my supervisor the form, asking for wage verifacation, hours worked, days missed, etc. My supervisor gave me back the form. It was not completed. What he told me was the second line supervisor told him ''they did not want to get involved.'' Is it not my right to have this information given to my insurance company if I signed an authorization for them to do so?

Assuming the accident was not work related, I am not aware of any law that requires an employer to complete the insurance form.
