Warn Notification form while on medical leave

While on medical leave, my employer informed me, at home, that i must sign a ''warn notification'' form. am i obligated in any way to do this? The form was mailed to my home.

Re: Warn Notification form while on medical leave

"WARN" forms are notifications of mass layoffs or terminations by an employer that will occur after 60 days. Your employer is simply trying to document the fact that they have provided you the required 60 day notice that you and your fellow employees are about to lose your jobs.
