I am trying to find out if there is a web site I can go to to get a sample writ of mandate concerning an administrative agency decision

You may go to your local law library and see if any of the "how to" books contain that information. You might also use a court's computer and see if you can locate a file that contains one and view the file. In any case, I suggest that you locate an attorney who practices administrative law.
