Signing your name for you is illegal. But it may not be worth taking legal action unless it has caused you some harm. You should make a written report to Human Resources Dep't or, if there is no HR, to whomever is in charge of employee matters. If that is the person who signed your name, make your report to their boss. Keep copies of everything. Keep a written record of everything that transpires regarding this matter, including any conversations.
