written warning from employer with false accusations

Are employers required to follow steps in writing written warning and threatening termination? I received a written warning accussing me of things that are not true. The accusations were general and have never been documented. My last review stated I was meeting requirements. One accusation stated I made derogotory statements about Hispanics. This is not true. Do I have any rights when it comes to accusations that are not documented? What steps do employers have to follow by law?
