Three written warnings before termination

I,as a manager, was terminated for what the company says was using abusive or profane laungage to a coworker. This didn't happen and I was never given the oppertunity to give any of my dtails. Is it ture that an employee must be written up 3 times for the incident before they can be terminated? If so what is the code number for this?
