There are no official forms. You would have to file a written motion with the court along with a proposed order. You might find a sample in a book at the law library.

The most common forms of contempt, however, are related to Family Law and/or Domestic Violence. If yours is that kind of case there are forms available. You can find them online at courtinfo.ca.gov, but you would be better off contacting the self-help center at your local superior court, since they can help you fill out the forms too.
