i need a itemization form to deduct damages to my property that tentant did.

my tentant left property and i want to deduct all damages towards her security deposit? is this possible what form and how to receive it.

Please refer to Civil Code section 1950.5, which requires that, within 21 days after the tenant vacates, you must mail or personally deliver to the tenant a copy of the itemized statement of what the security deposit has been used for and return any unused portion. There is no form. That code section also specifies how you may apply the security deposit and the other documents you're required to provide the tenant, including bills by third parties for repairs.
