i need a legal form stating my right to be reimbursed by landlord for repairs made and payed for by myself under the pre agreed deal made by landlord and tenent

You might simply write a letter to the landlord reminding him/her of the obligation to comply with the written agreement. If the landlord still does not, then your only recourse seems to be small claims court.
