What does it mean if you have a month to month rental agreement with a minimum tenancy of 12 months? How is that different from having a fixed lease for a year?

It means you have a badly drafted document. What it sounds like is a year lease with automatic conversion to a month-to-month tenancy at the end of the term, which is very common. Without reading the document, however, I can't say if that is the actual legal effect of the document.
