Notice to vacate
I wld lke to knw hw i obtain a cpy of the relief of forfeiture form to file frm a unlawful detainer judgemnt i was also unpropertly served with a unawful detainer pls help

I am not aware of any "relief from forfeiture" form. If it was a default judgment, you need to file a motion to set aside the judgement and request a stay of execution of the judgment. If you lost after trial, I believe need to file an appeal and bring a proper motion to stay enforcement of the judgment.
