proper forms to ask tenant to leave

i am renting a room out in my home

(i own) that i live in as well. What

forms do i need to give to legally let

them know i want them out by the

end of the 6 month lease which

turns in to a month to month

afterwards....and if they dont leave

by the end of the 30 days which form

do i give them after or do i just take

legal action?

You don't say whether there are other tenants, because the answer would be different.
