Can I rent out my house in California but 1) exclude the garage and 2) allow a secretary to come onto the premise through the backyard and allow her to work in an office that has no entrance to the house? Can these clauses be legally added to a lease?

You can have a lease agreement that allows for what you are proposing. You may want to consult with an attorney in your area to make sure your lease is properly drafted so as to protect you to the degree possible.
