How do I respond to an 'order to show cause re dismissal'??

As the plaintiff, do I file a response declaration and ask for a trial date?

It depends on why the OSC issued. An OSC re: dismissal generally issues because you failed to do something you were supposed to do, most commonly the plaintiff either has not served all the parties to the action and filed proofs of service, or the plaintiff failed to show up for a case management conference or other hearing. Without knowing what you failed to do, there is no way to tell you what to do to fix it.
