Legal Question in Legal Ethics in California

Contract Law

I would like to know if a lawyer has broken a code of ethics concerning a contract. I was retained to do work under his direction. He has submitted the invoices to the State for repayment but the State has denied his claim because the lawyer has not correctly sent the proper materials. He now feels since the project has been completed he does not need to submit the paper work gain because he feels it is a waste of time (money) on his part. How do I complain to the State Bar? Has he violated any code of ethics ? He promised to pay me out of the proceeds but has not taken the time to submit the paper work.
