Medical Records Privacy

My question is related to medical records privacy.

I authorized one entity to obtain copies of my medical records from a specified location. That entity then (without my consent) modified the form (altering the name of the initial specified location) and then submitted the form to multiple medical professionals; thereby obtaining unauthorized access to my medical records from multiple sources.

Is this legal?
