arbitration agreement physician/patient

do you have to sign this agreement in order to have the doctor treat you?

No. Except the doctor can refer you elsewhere for treatment if you don't. Most doctors will not insist. The malpractice carrier pushes the issue since it saves the carrier money when lawsuits happen.

There is a recission period. You can be seen by the doctor and then send in notice of cancellation of the arbitration agreement. The doctor cannot abandon you treatment, but, again he can pass you off to another qualified person.
