my 17 year old dougther was driving my car and had an accident, the car insurance will not cover because she was not in the policy yet. my question is I'm I resposible for her 18 year old companion medical bills?

It all depends if she was excluded on the policy. If she is not excluded on the policy then she would be covered. If she is specifically excluded on the policy then you would be responsible for the passenger's personal injuries up to $15,000 if you are the registered owner of the car. Also you would only be responsible if the accident was your daughter's fault. If the passenger has automobile insurance through any policy (such as her parent's policy) they may be able to get coverage for medical bills and possible uninsured motorist coverage under that policy.
