Message: Is there a form to answer on or do you just respond on blank paper and attach docs to it noting which question it answers?
In addition, I need to ask for a publication to loc a def for PI case I am In Pro Per what a nightmare. Is there a form somewhere or just start on blank plead paper with 26 lines a law clerk faxed me the format out of a book that I really cant read.

For form interrogatories you just submit your answers and objections to the questions you think are not proper (must be a legal basis). If you do not properly object than it is a waiver of an objection even if the question is improper. You also want to make sure with regard to medical questions you answer as fully as possible but ad the caveat that this is all you can remember at this time and they should check your medical records to make sure you have not left anything out.
Follow the same numbering system on the form you received, otherwise your answers will not make sense.
Write them out or type them and submit them to opposing counsel, (make yourself a copy and do not file them with the court, make sure you attach a proof of service showing you sent the document to opposing counsel)
As far as the publication - you must try to serve a defendant in person (process server, non-party, over 18), or by mail with acknowledgmen of receipt form (judicial counsel form you can find on california courts website (it is in the pulldown menu)only after you have satisfied all of the statutory requirements for service of process can you then request the court allow you to publish notice.

If you have a PI case and are in pro per, why is that? If your a plaintiff, have your run the case by any PI attorneys?
You'll find the answers to your discovery questions in the Code of Civil Procedure. Section 2025 is the basic provision. You also have to check the court rules, State and Local in the county in which you are litigating for format requirements.
