Legal Question in Personal Injury in California

I am fighting a case pro per and I am at the mandatory settlement/final status hearing stage. I have read that i will need to provide a settlement statement, but I am unsure of what exactly that would include. I am fighting a case that has been going on for 5 years, and I am suing for a slip and fall and an assult. If that helps.
