Form of White waiver

I have been told that in CA in order to keep settlement negotiations from being admitted as evidence, the parties must sign a form called a White waiver. Where can I find this form?

In California settlement negotiations are not admissable at trial as a matter of law.

No forms are necessary in California. See California Evidence Code section 1152. However, if there is a reason to suspect your opponent will attempt to introduce that type of evidence, you, or your attorney, may want to file what is known as a "motion in limine" before trial, seeking to exclude all evidence of such negotiations.
