I was rear ended, estimate damage to bumper ($700 to $1000) medical bill (physical therapy - neck, emergency room bill $2600.00) lost wages $60.00.
Insurance company offered to pay 2600.00 + 60.00 + 750.00 car + 500 Pain/suffering... Is this fair for pain and suffering?

No, they are low-balling you. They should be paying a minimum of $6,000 for pain and suffering, more like $9,000. Courts usually award between 3 and 5 times the out of pocket damages for pain and suffering in cases with significant injuries. In a low value case like yours, 2x would be the minimum, up to 3x.
