Suggestions for Doctor's written testimony

I was rear-ended and suffered a soft-tissue neck injury. I've had to file in Small Claims Court for medical damages. I will be being my Chiroprator's written testimony. My doctor usually asked what I want her to cover in any written reports, etc. Any suggestions regarding key points or information that should be in the wriiten affidavit? Should it be notarized?

Why did you "have to" file in small claims court? Surely your case is worth more than $7500 and any lawyer with a pulse would take it on a contingent fee basis. Sit down with a lawyer at once and discuss whether you need to dismiss and refile in superior court.

I agree. You made a big mistake taking this case to small claims Court. You are entitled to general damages which small claims court will not award you.
