You don't "add" a name to a deed. What you will need to do is prepare a new deed by which you will transfer title from the owner(s) to the new owners.

Unless you know precisely what you are doing, I strongly recommend seeking the services of an experienced real estate attorney to advise you concerning this matter, and to prepare whatever deed you eventually use.

When you hire an attorney to perhaps draw up your deed, you should also ask that attorney about the wisdom of adding someone. Depending upon what you are trying to accomplish, adding someone can be anywhere from wise and generous to downright foolish. If taking care of your heirs now rather than later is involved, please be advised that there are substantial income tax consequences to passing property by deed rather than by will or living trust. And so on.........
