I am bartering my Nevada timeshare as partial payment for a concrete driveway.

What form/s do I need and where can I get them?

You would need lawyer-drafted documents meeting the requirements of both Nevada and California law, that will cost, properly done, more than the driveway. There is nothing you can buy off the shelf and fill in a couple blanks. This is a kinky, one-of-a-kind deal. There is no form.
