If disclosure not checked on the box which says term of lease do not exceed one year if not checked do i still have to follow the terms of lease after 1year if i m month to month? ??

If I understand your question correctly, the answer is yes. A one-year lease, upon its expiration without renewal, and when the tenant pays rent and the landlord accepts it, automatically becomes a month-to-month rental on the same terms and conditions as the original, now expired lease, in all respects except for its duration. Civil Code section 1945.
