How do I file a consent or declination for filing the initial case management statement?

Your question makes no sense. No one needs to consent to filing a CMC Statement. Each party must file one; no one else consents. As for a "declination" there is no such thing in legal terminology.

If you don't file a case management statement, the court can sanction you, which involves you being ordered to pay a fine, and sometimes the other side's attorney's fees.
