What forms and information must be included to change John Doe to a defendant's actual name?

This is considered an "amendment to complaint" as distinguished from an "amended complaint" and is usually much simpler. However, the process varies from county to county. Some Superior Courts have a local form for designation or identification of Doe defendants. You'll probably need to look at your court's local rules, or ask the clerk, to find out about forms and processes. After the Doe defendant is identified for the record, you'll need to serve him or her with all papers and then file a proof of service.
